By proceeding to book a physiotherapy session on PhysigoCare, you confirm that you have read, understood, and agreed to the following consent terms and disclaimers. Acceptance of this consent is mandatory to complete your booking.
1. Nature of Physiotherapy Services
1.1 Physiotherapy services provided through PhysigoCare are rehabilitative, preventive, and supportive in nature.
1.2 Physiotherapy does not include: - Medical diagnosis - Prescription of medicines or injections - Emergency or critical medical care
1.3 Treatment outcomes may vary depending on individual health conditions, compliance, and clinical factors.
2. Informed Consent
2.1 I voluntarily consent to receive physiotherapy services from a qualified physiotherapist empanelled with PhysigoCare.
2.2 I understand that physiotherapy may involve physical movement, manual techniques, and exercises that may cause temporary discomfort.
2.3 I confirm that I have truthfully disclosed my medical history, current conditions, pain levels, and any contraindications.
3. Tele-Physiotherapy (Online Consultation) Disclaimer
3.1 Online consultations are conducted based on information provided by me without physical examination.
3.2 I understand the limitations of virtual assessment and accept that recommendations are based on visible and verbal inputs only.
3.3 I agree to follow instructions responsibly and stop exercises if pain or discomfort increases.
4. Home Visit Disclaimer
4.1 I confirm that my home environment is safe, clean, and suitable for physiotherapy treatment.
4.2 PhysigoCare and the physiotherapist shall not be responsible for: - Pre-existing household hazards - Injuries caused due to unsafe surroundings - Issues unrelated to physiotherapy treatment
4.3 I agree to provide a respectful and secure environment for the physiotherapist.
5. Emergency & Medical Referral
5.1 I understand that physiotherapy does not replace medical or emergency care.
5.2 In case of red-flag symptoms, the physiotherapist may stop treatment and advise medical consultation.
5.3 PhysigoCare is not liable for outcomes resulting from delayed medical intervention.
6. Data Protection & Privacy Consent
6.1 I consent to the collection, storage, and processing of my personal and medical data for: - Treatment delivery - Clinical documentation - Quality assurance
6.2 I understand that my data will not be shared with third parties except as required by law or for service delivery.
6.3 I consent to the use of anonymized data for internal analysis and service improvement.
7. Liability Disclaimer
7.1 Physiotherapy involves inherent risks, and I voluntarily assume responsibility for participation.
7.2 PhysigoCare acts as a healthcare service platform and is not directly liable for individual clinical judgment exercised by the physiotherapist.
7.3 Any claims arising from negligence or misconduct shall be addressed as per applicable law.
8. Consent Confirmation (Mandatory)
By clicking “I Agree & Proceed”, I confirm that:
· I am 18 years of age or older, or booking under guardian consent
· I have read and understood this consent
· I voluntarily agree to receive physiotherapy services through PhysigoCare
This consent shall remain valid for all bookings unless withdrawn in writing.
